Guidance on how to fill in the form and provide supporting materials:

1. *Required;

2. The forms of explanators and agents include, but are not limited to, individuals, companies, enterprises, social groups, etc., and the explanators are the original owners of personal rights, intellectual property rights and other legal rights. Agent refers to a person legally authorized by the right holder. If the right holder entrusts an agent to issue a notice, the agent must provide the power of attorney and other authorization documents of the right holder. Each different notice given by the agent shall be expressly authorized by the right holder separately;

3. Identification documents:

(1) Individuals: including valid ID cards, resident registers, passports, foreigners' residence permits, etc. If you provide a national ID card, please provide a copy of the front and back, and if you provide other documents, please provide a copy of the first page and personal page of the certificate (or a scanned copy of the original). The most commonly used valid ID card is a valid national identity card.

(2) Companies, enterprises or social organizations: Enterprises, institutions and social organizations need to provide a copy (or a scanned copy of the original) of the business license, public institution legal person certificate, social group legal person registration certificate and other documents within the validity period and affix the official seal.

4. Specific viewpoints for requesting restoration: In view of the facts and reasons for infringement raised by the complainant, it should be fully stated that there is no act complained of, or that the relevant act does not constitute infringement;

5. Appeal materials: including but not limited to copies of the preliminary proof materials that the specifier should provide as listed in the Guidelines for Infringement Complaints that do not constitute infringement.

(1) Proof of ownership: including but not limited to proof of ownership of intellectual property rights or the right to use the infringing links that have been frozen or blocked, the right of information network dissemination, the manuscript of creation, the timestamp of the creation of the work issued by an authoritative organization, the record certificate of the work, the letter of authorization, the authorization contract, and other valid ownership certificates that can prove that the respondent has the relevant rights;

(2) Proof that the post/content/comment/service/live broadcast provided by the respondent does not constitute infringement: including but not limited to the valid proof that the post/content/comment/service/live broadcast provided by the respondent does not infringe the personal rights, copyrights, trademark rights, patent rights and other rights of the right holder.

6. If you entrust another person to make a complaint, you should also provide a legal and valid power of attorney and the identity document of the trustee.
